A therapeutic mentor works one-on-one with a child or youth who, because of their behavioral-health needs, requires support and coaching to learn social skills, including better ways to communicate and get along with others. Services are provided in any setting where the youth resides, such as the home (including foster homes and therapeutic foster homes), and in the other community settings such as school, child care centers, respite settings and other culturally and linguistically appropriate community settings.
Through the local Community Support Agencies (CSA), Intensive Care Coordinators, InHome Therapy Clinicians or Outpatient Therapy Clinicians.
Service rates are payable under the benefit plans through any of the Managed Care Organizations for Mass Health (MBHP, BMC, Beacon Health Strategies, Network Health, or Neighborhood Health Plan). Private pay rates available.

In-Home Therapy Services are a structured, consistent, strength-based therapeutic relationship between a licensed clinician and the youth and family for the purpose of treating the youth’s behavioral health needs, including improving the family’s ability to provide effective support for the youth to promote his/her healthy functioning within the family. Interventions are designed to enhance the family’s capacity to improve the youth’s functioning in the home and community and may prevent the need for the youth’s admission to an inpatient hospital, psychiatric residential treatment facility or other treatment setting.
Primary care offices, mental health clinicians, schools, case workers, community organizations, faith leaders, parents/caregivers and families can make referrals directly to Family Continuity or through the local Community Support Agencies (CSA).

Family Continuity’s Individual Youth Support Program provides innovative and supportive mentoring services to youth. Specially trained mentors are matched with children and young adults with similar skills and interests. Mentors work in close collaboration with both the youth and their parents/guardians. The purpose of the program is to help youth to develop the competencies they need for developmentally appropriate functioning in the community.
Services are strengths-based and designed to have the individual and family make decisions for treatment planning.
Clients are referred by the Department of Mental Health.
There is no cost to families in this program.

Intake Coordinator
Family Continuity’s Youth Support Groups provide youth with therapeutic support services in a small group atmosphere. The purpose of this program is to provide psycho-education and therapeutic recreation activities to groups of youth that focus on social and emotional skill building, community integration, and educational and vocational preparation. Groups are designed to provide youth with opportunities to build skills, self esteem, and confidence in their communities. These Groups are led or co-facilitated by caring adults who are trained and supervised to facilitate growth and development in all group participants.
Snacks are provided.
Clients are referred by the Department of Mental Health.

Intake Coordinator
Offered out of our Lawrence and Hyannis offices, Family Continuity’s Family Systems Intervention Program provides flexible services to children and young adults with mental illness and their families. Our diverse outreach program has varying levels of intensity that provide different kinds of support. The purpose of the program is to help families and youth develop the skills and supports they need to promote family cohesion, successful community living and to improve their overall quality of life. Services are provided in the family’s home or in the community and individualized to each family’s needs.
The number of hours spent with a family is expected to increase in time of crisis to prevent hospitalization and to wane as the family acquires resources to resolve issues on its own.
Clients are referred by the Department of Mental Health.
