Family Continuity is committed to providing an Integrated Model of Care in Massachusetts, to this end, we provide clinicians in a variety of community settings such as including behavioral health and initial psychiatric assessment in local schools. Our School-Based Behavioral Health Centers (SBBHC) originated in response to area schools' increased need for behavioral health support. The goal of the SBBHC model is to reduce the demands on schools nurses, guidance departments, and teachers while increasing the level of support children receive throughout the school day by providing clinicians to work with children in the school. Additionally, we can provide psychiatric assessment and training for administrators on how to recognize trauma and identify children at risk.
Clinicians work in partnership with medical professionals to provide a bridge between these institutions and specialty behavioral healthcare when necessary. This continuity of care provides a holistic approach to the child's experience and reduces treatment barriers.
Currently, FC is providing services to the following school districts:
Central
- Northbridge, Uxbridge, Medway, Douglas, Mendon-Upton, Blackstone-Millville, Medway. and Bellingham High Schools and Middle Schools
Northeast
- Peabody Public School District
- Lawrence High School, Lawrence
Southeast
- Barnstable Middle School
