May 2015 Presidents Message1If an individual is presenting with urgent care needs, but not in need of hospitalization or screening, a referral can be made by calling our intake coordinator at the number below. Urgent care is designed to support individuals through crisis to a more stable and ongoing level of care. 

  • Client will be given an appointment to be seen within one business day of referral
  • Appointments can be up to two hours in length
  • Client will be moved to the top of the referral list for individual therapy, if not already in individual therapy
  • Clinician will make appropriate referrals for client
  • Clinician can also, if medically necessary, continue to meet with the client until individual therapy or other appropriate service appointment is secured
Referrals

Clients can call the clinic directly to self-refer. Referrals also come to the clinic through a variety of sources, including state agencies, ESPs, hospitals and other community services.

Cost

We accept MBHP insurance only for this service.

Family Continuity serves all individuals and families regardless of ability to pay. No one will be denied access to services due to inability to pay. Discounts for essential services are offered depending upon family size and income. You may apply for a discount at the front desk. Thank you.

FAMILY CONTINUITY HAS THE FOLLOWING POLICY FOR CHARGES FOR HEALTH CARE SERVICES

We will charge persons receiving health services at the usual and customary rate prevailing in this area. Health services will be provided at a reduced charge to persons unable to pay for services. In addition, persons will be charged for services to the extent that payment will be made by a third party authorized or under legal obligation to pay the charges.

We will not discriminate against any person receiving health services because of their inability to pay for services, or because payment for the health services will be made under Part A or B of Title XVIII (“Medicare”) or Title XIX (“Medicaid”) of the Social Security Act.

We will accept assignment under the Social Security Act for all services for which payment may be made under Part B of Title XVIII (“Medicare”) of the Act.

We have an agreement with the State agency which administers the State plan for medical assistance under Title XIX (“Medicaid”) of the Social Security Act to provide services to persons entitled to medical assistance under the plan.

 

 

Building strong communities requires collaboration, vision and cooperation. In order to provide the highest quality service, we believe it is imagesessential that we are not only connected to the communities we serve, but are active participants in working together with partners to promote mental wellness throughout our service area. To this end, we work together with municipalities, police departments and other providers to design programs that increase access to services and decrease stigma associated with mental illness. Some examples are:

Community Outreach Initiative Network (COIN) Collaborative

COIN is a ten town collaboration of municipalities under the jurisdiction of the Ayer District Court (ADC) that includes the communities of Ashby, Ayer, Boxborough, Dunstable, Groton, Littleton, Pepperell, Shirley, Townsend, and Westford. The goal of the COIN Collaborative is to connect citizens affected by mental health and/or substance use disorders to available resources, and divert them from the criminal justice system. This goal is being realized through funding from a DMH grant for the implementation of an Innovative Opiate Outreach and Jail Diversion Program. Currently, Family Continuity clinicians work alongside police departments to reach out to individuals in need of services. This collaboration diverts citizens from the criminal justice system by providing them the resources and support they need to get ongoing treatment. This program is run out of our Whitinsville office.  Blackstone Valley Connector

Thanks to a grant from the Greater Milford Community Health Network for CHNA 6, individuals in the Blackstone Valley in need of supportive services canIMG 7636 now go to the Blackstone Valley Connector. Promoting physical, mental and emotional health, staff at the Connector  provide jail diversion and intervention services to give vulnerable individuals support and keep them out of jail and engaged in a healthier way of life. Collaborating with five local police departments  (Blackstone, Millville, Northbridge, Sutton, and Uxbridge), police and other first responders can refer individuals and families directly to the Connector. The counseling staff at the Connector will then outreach and assist individuals to address their needs. This could involve anything from enrolling in a detox program, conducting a housing search, or connecting to domestic violence services. This program is run out of our Whitinsville office. Barriers such as language, lack of knowledge of available support systems, and the stigma associated with treatment create often insurmountable challenges for children and families in need. Family Continuity addresses these realities by going where the need is. Working with our community partners, we have clinicians in a variety of community settings ready to support individuals. These placements are tailored to the needs of each organization or setting and aim to increase access to care by giving people both simple, constructive resources as well as more intense therapy when needed. Whether it is providing groups to YWCA members or clinicians working with homeless families through Citizens for Adequate Housing, individuals and families are more readily able to access the help they need and work toward long term mental wellness. We currently provide tailored services to the following community partners:

  • Citizens for Adequate Housing

Family Continuity’s Family Support & Training Program provides services to parents/caregivers of a youth under age 21 in any setting where the youth resides. The staff, known as Family Partners, provides a structured, one-to-one, strength-based relationship between themselves and the parent/caregiver. The purpose of this service is tor esolve and ameliorate the youth’s behavioral and emotional needs by improving the parents’ /caregivers’ capacity to parent effectively.

Specific areas of support include:
  • Education to the parent/caregiver
  • Assistance in navigating the child-serving systems (DCF, education, mental health, juvenile justice, etc)
  • Fostering empowerment
  • Providing linkages to peer/parent support and self-help group
  • Assistance in identifying formal and informal community resources (e.g. after-school programs, food assistance, summer camps, etc)
  • Support, Coaching and Training for the parent/caregiver
  • 24/7 telephone support with program services available 7 days a week between the hours of 8am and 8pm
Referrals

Through the local Community Support Agencies (CSA), In-Home Therapy Clinicians or Outpatient Therapy Clinicians.

Cost

Service rates are payable under the benefit plans through any of the Managed Care Organizations for Mass Health (MBHP, BMC, Fallon, Beacon Health Strategies, Network Health or Neighborhood Health Plan). Private pay rates available.
